To Ascend a Hero, you must fulfill all of the requirements and pay the cost in coins. Requirements vary by Star Rating of the Heroes as listed below.
Regardless of Star level, all Heroes must fill all available Gear Slots and have unlocked all available Talents. Some Heroes require 6 items, and some others require less. You can always view the missing requirements straight from the Hero's inspector screen.
Go to Barracks, select the Hero you want to check, then tap on the "Gear" button at the bottom of the screen. The Ascension requirements are listed in the middle of the screen.
